To be eligible for the student rate, you must be a current full-time student. Please forward a copy of your student identification to wpregistration@iceaustralia.com in order to prove your status.

Note: The deadline for all registrations is Thursday 15 October 2009.
Registrations made after this date will incur a $AUD50.00 late registration fee.

DIGNITY CONSERVING THERAPY WORKSHOP
Places in Professor Harvey Chochinov's Dignity Conserving Therapy Workshop are limited and as a result places will be allocated through an application process.

The technique will be taught in two days of intensive inter-active training to experienced health care professionals who have strong communication and counselling skills.

During the registration process you will be asked to address the following criteria:
  1. Experience in counselling (e.g. list current qualifications)
  2. Indication of your current place of employment (palliative care or oncology fields will be given preference)
  3. Indication of how you believe the new skills learnt in this workshop will translate into patient care.
Applicants will then be contacted within 2 weeks of their application with the result. Successful applicants will then be issued with an invoice for payment which must be paid by 30 September in order for their place in the workshop to be confirmed.

Due to overwhelming interest we will consider every application for the Dignity Therapy workshop and a decision will be made on the merit of each application taking into account our limitations in numbers. Applicants will be notified within two weeks of the receipt of their application.

CANCELLATIONS AND AMENDMENTS
Registration and/or accommodation cancellations and amendments must be sent in writing either by mail, fax or email to ICE Australia Pty Ltd.

Registration cancellations received up to and including 29 September 2009 will receive a full refund, less AUD $150.00 administration fee and is payable upon cancellation. Registration cancellations received between 29 September 2009 and 15 October 2009 will receive a 50% refund (minimum cancellation fee of $150 is applicable). No refunds will be given for registration cancellations received after 15 October 2009.

As an alternative to cancellation, your registration may be transferred to another person before 29 September 2009 without receiving any extra cost penalty. Transfers made after 29 September 2009 will incur a AUD $20.00 transfer fee.

Accommodation will not be reserved without credit card details. Accommodation cancellations received after 29 September 2009 will be charged by the hotel according to their cancellation conditions.
