Please complete this form and bring with you to the Jupiters Hotel & Casino. Upon arrival at the venue, please proceed to the Pavilion Foyer where you registration can be processed.
Please see below for your appropriate registration fee, keeping in mind its closure date. To maintain your pre-registered conference rate you are required to pay within 7 days of registering, please keep this in mind if you are required to obtain prior authorisation.

Member Rate
To receive the member rate, you must be a member of the IQA or CCAA at the time of registration. You shall be asked for these details so please have them ready when registering.
Early Bird Registration
To receive the Early Bird rate you must register by Friday 30 July 2010.
Accompanying Person Registration
The Accompanying Person program includes the following tours:

Date: Thursday 28th October
Time: 11.00am - 3:00pm
Venue: Tour of Q1 Lookout & lunch at Oscars Restaurant, Burleigh Heads waterfront
Orica Partners Day
Date: Friday 29th October
Time: 9:30am - 3:00pm
Venue: Mystery location
Once again Orica will host one of the partners tour.

Single Day Registration
It is a condition of the single day registration that entry is permitted for that day only. If you are attending more than one day, the registration is no longer valid and a full registration must be purchased.
The deadline for all registrations is Friday 8 October 2010.
Registrations made after this date will incur a $AUD50.00 late registration fee. |
